Position Title: Co-response Social Worker - Salt Lake City Fire Department Job Description: The Salt Lake City Fire Department is looking to add you to our team! About the Position: The CHAT program seeks to provide personalized services to individuals who will not benefit long-term from reliance on the 911 emergency system. This may be achieved by connecting the individual to education and resources needed to prevent local Emergency Departments from becoming de-facto, primary care providers, services which hospital ERs are ill-equipped to provide practically or effectively. CHAT will identify potential patients, work with them to determine needs, and connect them to resources that may be better suited to provide for those needs. CHAT will combine social workers with emergency service personnel to care for both physical and mental health. In addition, CHAT will develop a patient-care plan focused on the whole person and connect them to appropriate services that may address their specific primary care issues. The work schedule would be Thursday-Saturday 0730 hours - 2030 hours (2130 hours on Thursday). The position and schedule may change depending on the needs of the department. This position may be filled as a Social Service Worker/Licensed Substance Use Disorder Counselor or a Licensed Clinical Social Worker/Clinical Mental Health Counselor depending on the candidates’ qualifications. Position Salary Range: Social Service Worker/Licensed Substance Use Disorder Counselor N21 – Non-exempt, full-time: $22.93/hr. - $31.29/hr. Licensed Clinical Social Worker/Clinical Mental Health Counselor E26 – Exempt, full-time: $60,941/yr. to $83,111/yr. The established market rate for this position is $83,111/yr. Key Responsibilities: Acts as a bridge between the individual, department, community, community agencies and other helping professionals to coordinate services and the best route of care for the person looking for assistance. Monitors and documents any and all care given to the individual. Routinely assesses the patient and family responses to the interventions and services, while also measuring treatment plan effectiveness. Prepares and retains timely correspondence, reports, patient progress, and/or other documentation. Gives debriefing to responders following traumatic incidents as requested by the department. Demonstrates and maintains a high standard of confidentiality and sensitivity to the persons and/or families on cultural, ethnic, racial, socioeconomic issues and diversity. Assists, educates, informs, and empowers under-represented populations in the community, including vulnerable communities, refugees and immigrant populations. Attends staff/staffing meetings and other meetings as assigned. May be required to drive a vehicle to and from various locations throughout the community and provide transportation for patients as needed. Rides with officers to assist in co-responder model serving those in crisis and working with top utilizers of 911 services. May perform other duties as assigned. Additional Licensed Clinical Social Worker/Clinical Mental Health Counselor Responsibilities: Provides clinical and intensive case management services, including, but not limited to the following: biopsychosocial assessments, treatment and service planning, clinical/crisis counseling, consultation, support, information, and appropriate referrals to other community agencies. Conducts individual, family and/or group therapy sessions in accordance with the established treatment plan and provide crisis intervention when necessary. Additional Social Service Worker/Licensed Substance Use Disorder Counselor Responsibilities: Provides direct case management services, including but not limited to the following: crisis intervention, ongoing assessment, goal setting, problem solving, service planning, consultation, support, information, intensive case management and appropriate referrals to other community agencies. May be required to complete home or site visits as required by the department. Minimum Qualifications: Demonstrated ability to work independently as well as take initiative and work collaboratively as a part of a team. Possess interpersonal skills, active listening skills, critical analysis, and written and verbal communication skills. Demonstrated independent judgment to assess and meet citizen needs. Ability to have meaningful outcome oriented dialogue with citizens, families, community agencies and professional schools in coordinating care and services for citizens. Demonstrated appropriate boundaries with citizens and/ or their families while providing adequate services to meet citizen needs. Possession of a valid state driver license or Utah driving privilege card (or the ability to obtain one within 30 days of employment). Successful completion of a drug screen if incumbent has not completed it as a Salt Lake City employee. Additional Licensed Clinical Social Worker/Clinical Mental Health Counselor Qualifications: Master’s Degree in Social Work, Mental Health Counseling, or a closely related field from an accredited college/university. Current Licensure with the State of Utah as a Licensed Clinical Social Worker, Clinical Mental Health Counselor, Certified Social Worker, Associate Clinical Mental Health Counselor, Certified Social Worker Intern or the ability to obtain a state licensure within 6 months of the hire date. Additional Social Service Worker/Licensed Substance Use Disorder Counselor Qualifications: Current Licensure with the State of Utah as a Social Service Worker, Advanced Substance Use Disorder Counselor, Licensed Substance Use Disorder Counselor or Certified Substance Use Disorder Counselor OR five years of directly related work experience in social work. Desired Qualifications: Bachelor’s Degree in Social Work from an accredited college/university and three to five years of related work experience, which included assessing the needs of a diverse population. Experience working collaboratively with community agencies. Experience working in both substance abuse and mental health treatment fields. Fluent in oral and written Spanish communication. Mental Health Officer Certification. The ability to supervise Certified Social Workers, students and/or Associate Clinical Mental Health Counselors, if needed. Working Conditions: Must stand, walk, or sit uncomfortably for extended periods. Moderate exposure to elements including, but not limited to, heat, cold, dampness, fumes, noise, dust, or grease. Considerable exposure to stressful situations as a result of human behaviors. In field operations, exposure to stress as well as occupational hazards is of considerable magnitude. Career Ladder: After successful performance as Social Service Worker, during which incumbent demonstrates ability to perform assigned tasks with minimal supervision and has completed a Master’s Degree in Social Work, Mental Health Counseling, or a closely related field from an accredited college/university with current Licensure with the State of Utah as a Licensed Clinical Social Worker, Clinical Mental Health Counselor, Certified Social Worker, Associate Clinical Mental Health Counselor, Certified Social Worker Intern or the ability to obtain a state licensure within 6 months of the hire date, he/she may be promoted to Licensed Clinical Social Worker/Clinical Mental Health Counselor.
