What You Will Do As the Care Coordinator in Counseling Services, you will play an important role in helping students navigate behavioral health care and stay connected to the support they need. Working within our stepped‑care and walk‑in access model, you will help students identify the appropriate level of care, address barriers to treatment, and navigate campus and community resources. Provide behavioral health screening, triage, case management, care coordination, crisis support, and resource navigation. Maintain a case management caseload and provide ongoing follow‑up and continuity of care for students with complex behavioral health needs. Help students navigate barriers to care, including insurance, transportation, financial, scheduling, and access concerns. Coordinate warm handoffs and referrals to Counseling Services, Health Services, community behavioral health providers, hospitals, and other campus and community resources. Support students transitioning from psychiatric hospitalization or other higher levels of care by coordinating follow‑up, treatment connections, and continuity of care. Serve on the university CARE Team and conduct mandated behavioral health assessments referred by the Dean of Students. Collaborate with campus and community partners to develop coordinated support plans for students of concern. Build and maintain relationships with community behavioral health providers and strengthen referral pathways for students. Provide mentorship and training to social work practicum students as assigned. Facilitate or co‑facilitate psychoeducational workshops and support groups focused on student well‑being, coping skills, and connection to resources. Work closely with Counseling Services clinicians and other members of the care team, seeking clinical consultation and escalating concerns when appropriate. This position functions under the clinical supervision of a Licensed Clinical Social Worker (LCSW) and does not provide ongoing psychotherapy. FLSA: Non‑Exempt Minimum Acceptable Qualifications –Bachelors degree. -A total of one (1) year (12 months) in education, training and/or work experience in the area of specialization inherent to the position. (Note: Masters Degree in an area consistent with the duties of the position may be substituted for one (1) year of work experience.) AND Years can be concurrent -1+ years experience providing behavioral health case coordination/management, care coordination, and/or resource navigation with a strong knowledge of community mental health resources, insurance systems, and referral processes. Preferred Qualifications Experience working in a college or university counseling center, student affairs, behavioral health, or integrated healthcare setting serving emerging adults. Demonstrated experience conducting crisis assessment, intervention, safety planning, and coordinating follow‑up care for individuals experiencing acute mental health concerns. Experience serving on a multidisciplinary CARE Team, Behavioral Intervention Team (BIT), or similar threat assessment and student support team. Knowledge of the stepped care model and experience connecting individuals with the most appropriate level of behavioral health treatment and community resources. Experience collaborating with campus and community partners, including disability services, dean of students, academic advising, student health services, hospitals, residential life, and community behavioral health providers to coordinate comprehensive care. Experience working with diverse student populations and demonstrated commitment to providing culturally responsive, trauma‑informed, and inclusive services. Experience providing continuity of care care following hospitalization or other higher levels of treatment.
