The Outpatient Therapist plays a vital role in our mission to deliver coordinated, individualized care to our communities.As an Outpatient Therapist , you will provide:* **Case Management Services**: Coordinate and manage services for clients, ensuring they have access to necessary resources and support.* **Service Plans**: Develop individualized, written service plans to address clients’ specific needs and goals.* **Crisis Intervention**: Provide immediate support and intervention during mental health crises.* **Evaluations and Assessments**: Conduct psychological evaluations and write diagnostic assessments, including those for behavioral, substance abuse, and economic self-sufficiency issues.* **Facilitate Wraparound Meetings**: Organize and lead team meetings to ensure coordinated care among providers and natural supports.This description outlines the general scope and level of work for this role.
