- Make an impact on seeing the underserved population - Seeking a Board-Certified Internal Medicine Physician - Bi-lingual English / Spanish - Outpatient responsibilities only - Full-time permanent position - Monday " Friday - No nights, no weekends - Small patient panels for consistency and focus - 12-15 patients per day -Value-based care model and competitive bonuses are structured to reward outcomes -Provide personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest - Location: Elgin, IL - 35 miles NW of Chicago along the Fox River.
