Mobile Nurse Practitioner – San Fernando Valley & Antelope Valley, CA Full-Time | Mobile/Field-Based | Home-Based Patient Care | Excellent Work-Life Balance A nationally recognized healthcare organization is seeking a Mobile Nurse Practitioner to provide acute, post-acute, and transitional care to medically complex and recently discharged patients throughout the San Fernando Valley and Antelope Valley, California . You’ll care for patients who are transitioning home following hospitalization or skilled nursing facility stays, as well as individuals managing complex chronic conditions. This field-based position provides the opportunity to truly understand each patient's environment, support system, and unique healthcare needs. You’ll have significant clinical autonomy while remaining closely connected to physicians, nurses, care managers, pharmacists, social workers, behavioral health specialists, and other members of the care team. Compensation & Benefits $140,313–$178,968 annual base salary Bonus eligibility based on individual and company performance Comprehensive Health, Dental & Vision Insurance Paid Time Off and company holidays Mileage reimbursement for business travel Employer-provided malpractice coverage Retirement savings opportunities Professional development and career growth support Supportive interdisciplinary care team Opportunity to provide meaningful, relationship-centered healthcare Schedule Full-time Field/mobile position Home visits throughout the San Fernando Valley and Antelope Valley Primarily focused on acute, post-acute, and transitional care Opportunity for significant clinical autonomy How You'll Make an Impact As a Mobile Nurse Practitioner, you will: Provide acute, post-acute, and transitional care directly in patients' homes Evaluate medically complex and recently discharged patients Perform comprehensive physical assessments Diagnose and treat acute and chronic medical conditions Develop individualized treatment and care plans Manage medications and coordinate appropriate follow-up care Identify potential complications and intervene early Help reduce unnecessary hospitalizations and readmissions Educate patients and families regarding disease management and preventive care Coordinate care with physicians, nurses, pharmacists, social workers, and other providers Communicate with hospitals and skilled nursing facilities during patient transitions Maintain accurate and timely clinical documentation Utilize clinical judgment and work independently in a field-based environment Participate in an interdisciplinary approach to patient care What We're Looking For Minimum Qualifications Master of Science in Nursing ( MSN ) Adult/Geriatric Nurse Practitioner certification Active, unrestricted Registered Nurse (RN) license Active, unrestricted Nurse Practitioner license in California Minimum of 1 year of Nurse Practitioner clinical experience Strong assessment, diagnostic, and care management skills Excellent communication and interpersonal skills Ability to work independently while collaborating with a multidisciplinary team Valid driver's license Reliable transportation for home visits Current TB test or willingness to obtain prior to employment Join a Team That Brings Healthcare Home The best Nurse Practitioners want more than a traditional clinical position.
