In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Bridgeport, CT

Role Overview Signify Health, part of CVS Health, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations in the comfort of patients' homes. Key Responsibilities Conduct in‑home visits lasting about 45 minutes, including medication and medical history review, physical evaluation, and diagnostic tests as ordered. Use an iPad and clinical workflow; not required to prescribe medication, order lab tests, or alter current treatment regimens. Work flexibly to travel locally and within licensed states, and conduct virtual visits as needed. Provide Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, bone density screening, etc. Build trusted relationships, refer patients to appropriate care providers, and connect patients to the right care. Required Qualifications Active, unrestricted NHS or PA license(s) in the coverage area(s). Board certification required; multi‑state licenses preferred. Ability to cover local primary locations such as Bridgeport and surrounding area. Preferred Qualifications Multi‑state license(s). Compensation and Hours Part‑time: 29.5 hours per week. Pay for each completed In‑Home Health Evaluation ranges from $46.03 to $99.14 per hour, depending on experience, education, geography, and other factors.
