As a team member of our Senior Community Care (SCC) product, we work with a team to provide care to patients at home in a nursing home, assisted living for senior housing. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs). Standard Hours and Travel Standard Hours: Monday‑Friday 8:00 a.m. to 5:00 p.m. EST This is a field‑based position traveling to facilities throughout Cuyahoga County (i.e., primarily Western Cleveland, Parma, & North Royalton).
