Peer Mentor
Children's Hospital Los Angeles (CHLA)
Schedule Per-diem, 4pm on Tuesdays; mornings on Saturdays. Other hours flexibly scheduled per department needs. Position Summary The Peer Mentor for CHAT provides peer support to adolescent and young adult patients to help them gain self-advocacy and self-efficacy skills as well as skills to manage their health care needs. Facilitate and support access to CHLA and community resources to patients and families. Serve as a facilitator between patients, families, and their representatives across the enterprise. CHAT is accountable for implementing an institution-wide system for healthcare transition while patients are at CHLA and for transfer of care from CHLA to adult providers. Qualifications/Requirements Experience as a patient in a children’s hospital setting. Bilingual language capacity (Spanish) preferred.
