Job Summary:


The Therapy/Rehabilitation Occupational Therapist (OT) in Home Health provides skilled occupational therapy services to patients in their homes, focusing on improving daily living activities and functional abilities. This role requires licensure in both Washington and Oregon, with experience in home health settings, and involves managing a caseload of patients with diagnoses such as dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke. The OT collaborates with interdisciplinary teams, completes OASIS documentation, and delivers care aligned with national patient safety goals and home health standards.

Responsibilities:

• Provide 1:1 occupational therapy visits to up to 6 patients per day in home health settings.
• Perform patient assessments and develop individualized treatment plans focused on daily living and functional activities.
• Document patient care and outcomes using EPIC Remote Client, Rover app, and OASIS documentation tools.
• Manage patient caseload efficiently while minimizing travel and windshield time within a maximum daily radius of approximately 60 miles.
• Collaborate with interdisciplinary teams to ensure coordinated patient care and adherence to national patient safety goals and core measures.
• Maintain compliance with home health billing and documentation standards.
• Adhere to facility policies regarding attire, documentation, and patient care protocols.

Required Skills & Certifications:

• Active Occupational Therapy licensure in both Washington and Oregon (WA license preferred in hand; OR license can be pending).
• Minimum of 1 year of skilled home health occupational therapy experience.
• Current Basic Cardiac Life Support (BCLS) certification.
• Experience with OASIS documentation and home health clinical standards.
• Valid driver's license and reliable personal transportation for patient home visits.

Preferred Skills & Certifications:

• Knowledge of national patient safety goals and core measures.
• Experience with billing processes in home health care.
• Familiarity with managing patients with acute and chronic disorders affecting functional activities.
• Proficiency in EPIC Remote Client and Rover app for clinical documentation.

Special Considerations:

• Must hold or be eligible for licensure in both WA and OR states; both licenses are required for the role.
• No weekend shifts or on-call requirements.
• Facility cannot accommodate 4x10 or 3x12 shift schedules; standard 8-hour day shifts only.
• Traveler expected to have own transportation; travel radius may be up to 60 miles per day with grouped patient visits to minimize drive time.
• Facility may call off up to one shift every two weeks.
• Remote charting required using laptop and mobile applications.
• Business casual attire required; scrubs allowed but no jeans.

Scheduling:

• Standard 8-hour day shifts.
• Shift assignments do not include weekends or on-call duties.
• Block scheduling and floating between units are not accommodated.
• Guaranteed hours with potential for up to one shift call-off per two-week pay period.
