Work collaboratively with nurses, medical doctors, case managers, and rehabilitation professionals to maximize recovery outcomes. Guide patients and their families on therapeutic objectives, discharge procedures, and at-home exercise routines. Accurately document evaluations, treatment protocols, and patient progress according to facility rules and regulatory standards. Engage in care coordination through participation in team rounds and interdisciplinary meetings.

Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution. Current state licensure as a Physical Therapist. Preferred CPR certification. Additional specialized certifications (neurological, geriatric) highly regarded but not mandatory. Strongly preferred past experience in a clinical setting. Aptitude for treating patients presenting complex medical and mobility concerns. Excellent skills in communication and teamwork. Familiarity with digital charting and clinical documentation systems.
