Position: Assistant Director of Nursing/Wound Care Nurse
Location: Queens, NY
Shift: Full-Time | 8:00 AM – 4:00 PM
Salary: $125,000–$150,000 annually

Union Plaza Care Center is seeking an experienced and compassionate Assistant Director of Nursing/Wound Care Nurse (RN) to join our skilled nursing team. This is an excellent opportunity for a clinically strong nursing leader who is passionate about wound management, resident outcomes, staff development, and delivering high-quality, resident-centered care.

Key Responsibilities

• Assist the Director of Nursing with the planning, coordination, and oversight of daily nursing department operations.

• Assess residents admitted with existing wounds and residents who develop wounds during their stay.

• Collaborate with physicians and the interdisciplinary team to develop and implement individualized wound care treatment plans.

• Lead and participate in weekly wound rounds, including wound assessments, measurements, treatment recommendations, and documentation.

• Complete and maintain weekly wound tracking reports and promptly report non-healing or worsening wounds to physicians and appropriate clinical leadership.

• Track, analyze, and distribute weekly decubitus ulcer statistics.

• Provide in-service education and training to nursing staff regarding wound care best practices, prevention, documentation, and treatment techniques.

• Prepare quarterly Quality Assurance (QA) reports and participate in quality improvement initiatives.

• Observe and evaluate dressing changes to ensure proper technique and staff competency.

• Support nursing staff with clinical guidance, education, and resident care needs.

• Educate residents and families regarding wound care, prevention, and treatment as appropriate.

• Promote compliance with facility policies, New York State regulations, and applicable federal requirements.

• Participate in ongoing wound care education and professional development to remain current with industry standards and best practices.

Requirements

• Active New York State RN license.

• Wound Care Certification required.

• Infection Control Certification preferred.

• Minimum of 1 year of nursing home or long-term care experience preferred.

• Previous nursing leadership or supervisory experience preferred.

• Strong clinical assessment, documentation, communication, and organizational skills.

• Knowledge of wound care practices, infection prevention, and skilled nursing regulations.

• Ability to lead, educate, and support nursing staff in a fast-paced long-term care environment.
