Physical Therapist

Role: Physical Therapist providing personalized, in-home and community-based care to older adults. Responsibilities Conduct evaluations, develop individual treatment plans, and deliver evidence-based therapy to enhance mobility and independence. Perform one-on-one therapy sessions in private residences and senior living communities. Document patient progress accurately in electronic medical records. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Utilize telehealth and virtual care to extend access and flexibility. Qualifications & Skills Valid PT license or eligibility in practice state(s). Degree from an accredited Physical Therapy program. Proficiency in computer and EMR use. Current CPR certification. Key Requirements Ability to perform physical tasks such as lifting, repositioning patients, and extended walking/standing. Willingness to drive to patient locations and wear PPE as needed.
