Registered Nurse - Ambulatory (Pediatrics)

Overview Performs an age‑specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment to coordinate patient care and direct the patient‑care unit team. Responsibilities Conducts patient interviews, explains policies and procedures, reviews the patient’s chart, and answers questions courteously. Assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health status; determines mobility, sensory deficits, prosthesis use, skin condition, and pain level; and adjusts the plan of care accordingly. Assesses the patient’s psychosocial health status, elicits perception of medical/nursing care, identifies coping mechanisms, knowledge level, and ability to comprehend, and identifies cultural or ethnic requirements to reduce anxiety and ensure satisfaction. Communicates and documents psychological status and care plan for support groups, counseling services, and social services. Identifies patient outcomes, develops criteria for measuring outcomes, identifies actual or potential patient problems, and determines the need for teaching based on psychosocial and developmental assessment. Develops patient outcome statements and establishes individualized patient goals. Identifies care activities and establishes priorities to achieve expected outcomes. Coordinates the cost‑effective use of supplies, equipment, and medication, documents the plan of care, and collaborates with physicians and other health team members. Implements the plan of care, maintains vigilance to ensure safety precautions are followed, administers prescribed treatments, uses equipment properly, provides emotional support, applies scientific principles to procedures, reassesses patients and revises the plan of care as needed, and documents all observations and interventions. Acts quickly during emergencies and provides a calm, quiet, restful atmosphere. Demonstrates the ability to measure the effectiveness of care, performs variance analysis related to outcome data, and designs, implements, and evaluates systems to improve care. Collaborates with other care team members in planning and carrying out treatment regimens, and provides direction to team members. Accurately interprets and implements the treatment regimen prescribed by the physician, assists during treatments and diagnostic procedures, updates the physician on changes in condition, and delegates assignments appropriately. Performs grade I‑IV decubitus care, preventive skin care measures, simple dressing applications, warm and cold compresses, irrigations, and other skin‑care procedures as required. Administers medications correctly and safely, identifies medication action, dosage, side effects, and interactions, meets the medication administration examination standard, and demonstrates preparation of local solutions. Participates in patient and family education, explains the patient’s condition, records and verbalizes pertinent information, utilizes available teaching resources, plans early discharge and appropriate referrals, and evaluates the effectiveness of teaching through patient and family feedback. Qualifications Graduate from an accredited School of Nursing. Bachelor’s Degree in Nursing, preferred. Enrollment in an accredited BSN program within two years of job entry, with the BSN degree to be obtained within five years. Current license to practice as a Registered Professional Nurse in New York State required. Specialized certifications as needed. Previous pediatrics experience strongly preferred.
