About the Role: Intake Case Manager

As an Intake Case Manager, you will be the first point of contact for individuals seeking support through our interconnected ecosystem of care. You’ll play a vital role in assessing needs, connecting people with resources, and helping them navigate their journey toward healing, rebuilding, and thriving. This position is perfect for someone with at least 1 year of experience in case management or a related field who is eager to make a tangible impact.

Key Responsibilities

As an Intake Case Manager, your day-to-day responsibilities will include:

• Conducting intake assessments to understand the unique needs of individuals and families.
• Developing personalized care plans by identifying appropriate services and resources.
• Coordinating with internal teams and external partners to ensure seamless service delivery.
• Building trusting relationships with clients to support their journey toward stability and independence.
• Maintaining accurate and confidential records of client interactions and progress.
• Advocating for clients by connecting them to opportunities in mental health, housing, education, employment, and more.
• Monitoring and following up on client progress to ensure long-term success.

Required Skills and Qualifications

We’re looking for someone who brings the following to the table:

• Experience: A minimum of 1 year in case management, social work, or a related field.
• Communication Skills: Excellent verbal and written communication skills to engage with clients and collaborate with team members.
• Empathy and Compassion: A genuine desire to help others and the ability to build strong, trusting relationships.
• Organizational Skills: Strong attention to detail and the ability to manage multiple cases effectively.
• Problem-Solving Abilities: Creativity and resourcefulness in finding solutions for clients’ unique challenges.
• Team Player: Willingness to work collaboratively in a dynamic, mission-driven environment.
