What You Should Know About the LPN Team Float Role:

• This is a DRIVING position and will travel to see patient in both homes and facilities within Huber Heights OH and surrounding areas
• This is a FT day shift position, 5-8hr shifts per week with an every 6th weekend rotation and 1-2 holidays per year
• We provide superior care and superior services to patients at their end of life journey . Only those who have a heart for hospice will succeed

The LPN/RN Float Essential Duties:

The LPN Team Float provides quality nursing care to patients in a manner that meets the patient and family needs working under the direction and supervision of an RN Care Manager.

Qualifications:

LPN's - Registered LPN with the State of Ohio without any board actions. One year of previous nurse experience.

• CPR Certified
• Certification in Hospice & Palliative Medicine (CHLPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage
