Job Title TASC Care Manager Location New Hanover County Wilmington NC Hours 40 hoursweek Full Time M F Non Exempt Job Description Coastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to join our team This position offers a unique opportunity to support justice involved individuals by providing case management and service coordination within the TASC Treatment Accountability for Safer Communities program The ideal candidate will collaborate with community partners justice systems and service providers to help individuals access treatment and recovery services We have this specific opportunity located in New Hanover County Wilmington NC Primary duties administering psychosocial screeningsassessments coordinating linkage to behavioral health treatment as well as medical educational and vocational services as needed and providing ongoing professional care management services for the justice involved population REQUIRED EDUCATIONAL QUALIFICATIONS Education Bachelors Degree or higher in a human services or related field Certifications Preference given to individuals with North Carolina Peer Support Specialist certification and a minimum of 2 years of experience andor individuals registered with the North Carolina Addiction Specialist Professional Practice Board NCASPPB Experience Experience Knowledge of recovery harm reduction and wellness based concepts May have lived experience with recovery andor the justice system Required Skills Strong communication organizational and time management skills Ability to work independently and manage a caseload effectively Proficiency in the Microsoft Office Suite and similar platforms Proficiency in using electronic health record systems and remote collaboration tools ie Teams Zoom etc Demonstrated ability to collaborate with diverse stakeholders including legal systems healthcare providers and social services Salary & Benefits Competitive salary based on experience 42000 45000 Comprehensive medical dental and vision insurance Educational loan repayment programs & Career growth Retirement savings plan401K Paid time off programs rollover hours 14 paid holidays Employee engagement activities resource groups and diversity events EEO STATEMENT Coastal Horizons is an Equal Employment Opportunity EEO employer All qualified applicants will receive consideration for employment without regard to race color religion sexual orientation gender gender identity and expression national origin age disability or protected veteran status We celebrate diversity and are committed to creating an inclusive environment for all of our associates.
