Licensed Practical Nurse (LPN) - Long Term Care

Location: Rochester, NY

Assignment Duration: This is a 13-week contract position.

Schedule: Rotating shift schedule, including weekends, holidays, on-call, callback, and charge per unit needs.

Job Description: We are seeking a dedicated Licensed Practical Nurse (LPN) to provide care for older adults in a long-term care environment. This role requires a compassionate approach to caring for residents, primarily geriatric individuals, focusing on their dignity and respect. The nurse will manage medication administration and oversee the work of Certified Nursing Assistants (CNA) in the facility.

Responsibilities:

• Provide direct patient care, including medication administration.
• Oversee and assist CNAs in delivering quality care to residents.
• Conduct assessments and monitor residents' health status.
• Document patient care services and maintain accurate records.
• Communicate effectively with patients, families, and the healthcare team.

Required Experience / Certifications / Licensure:

• Current New York State LPN license.
• BLS certification from the American Heart Association.
• Minimum of 2 years of experience in a long-term care setting.
