Certified Nurse Aide

PURPOSE:To provide routine daily nursing care and services that support the care delivered to patients/residents requiring critical access acute care, in accordance with the established nursing care procedures and as directed by the supervisor.ENVIROMENT:Lincoln Community Hospital and Nursing Home is a 15-bed Critical Access Hospital co-located with 35-bed Long Term Care Unit. The facility employs 130-200 employees. The incumbent will manage both areas.ESSENTIAL DUTIES AND RESPONISILITIES:Provides care as directed by the professional nurse to patients/residents requiring long-term, rehabilitative care or restorative care.Reports observations and other pertinent information related to the care of the patient/resident.Documents objective information related to patient/resident care.Assists with registration of patients/residents in the computer.Provides services that support the care delivered to the patient/resident.Provides input in the formulation and evaluation of standards of care.Participates in the development of unit objectives.Participates on the identification of staff educational needs.Conducts self in a professional manner in compliance with unity and facility policy.Participates in the quality assessment and improvement process and activities.Performs other related duties as assigned or requested.EXPOSURE RISK:The Certified Nursing Assistant (CNA) is at high risk for exposure to blood and body fluids or other potentially infectious materials.SUPERVISION RECEIVCED:Receives administrative supervision from the Director of Nurses, or the Assistant Director of Nursing. May receive functional supervision from registered or practical nurses working on the unit.SUPERVISION EXERCISED:Exercises functional supervision, responsibility, and accountability necessary for carrying out assigned duties.MINIMUM QUALIFICATION:Graduation from high school.Previous nursing assistant experience or student nurse preferred.Completion of certification program for nursing assistants preferred.Nursing Assistants can work for 4 months following the completion of a class before testing.Must obtain BLS w/in 6 months of hire and maintain per certification requirements.MINIMUM PERFORMANCE STANDARDS:Performance in the following areas is acceptable when:General Patient/Resident Care:Patients/residents are identified by the use of the identification band prior to administering treatments, serving meals, etc., as necessary.Patient/resident is approached in a kind, gentle and friendly manner. Respect for the patient’s/resident’s dignity and privacy is consistently provided. Appropriate responses to requests are provided.Maintenance of Supplies/Equipment:All equipment is operated in a safe manner and the only equipment utilized that which previous training of use has occurred.Defective equipment is reported to the Charge/Staff Nurse.Vacant rooms are neat in appearance.The nurses’ station is maintained.Supplies are maintained at par levels and are arranged in an organized manner.Nurses’ station is neat and orderly.Full waste containers are emptied.Equipment is accounted for on rounds.Ice containersGeri-chairs, if applicable.Over-bed tables.IV poles, if applicable.Stretcher lift, if applicable.Cleaning and maintenance occurs according to schedule.IV or tube feeding pump poles, tube feeding infusion pumps, and suction machines are wiped down PRN.Utility room is checked routinely for supplies that needs to be returned to the appropriate department, meal trays requiring return to dietary, and linen that requires removal to the laundry.Safety hazards are removed from unit floors (liquids spills, food, paper, equipment cords, etc).Nourishment refrigerator is cleaned and defrosted once a month.The patient’s/resident’s environment is safe and clean.Individual patient/resident supplies are re-stocked as indicated.Adherence to Facility Procedures:Procedures are performed according to established method in the procedure manual.Body substance precautions and other appropriate infection control practices are utilized with all activities.Safety guidelines established by the facility (i.e., proper restraint and side-rail use) are followed.All accidents or incidents observed are reported on the shift that they occur.Documentation:All entries on charts, notes, flow sheets, etc., are recorded in an informative and descriptive manner.Vital signs are recorded accurately on the appropriate form.Intake and output is accurately recorded on the appropriate form.Patient/resident weights are recorded on the appropriate form.Nursing care flow sheet (if applicable) is maintained.Documentation of restorative program and progress towards goals.Communication:All changes in a patient’s/resident’s condition are reported as soon as possible to the Charge/Staff Nurse.Staff meetings are attended, if on duty. Minutes are read and initialed if not on duty.Patient/resident/problems are reported to the Charge Nurse.Professionalism:Decisions are made that reflect both knowledge and good judgment.Awareness of own limitations is evident and assistance is sought when necessary.Dress code is adhered to.Committee meetings (if assigned) are attended. Reports related to the committee are given during staff meetings.Responsibility is taken for own professional growth.
