Clinical Lead Nurse Practitioner/ Site Physician Assistant

Position: Clinical Lead Nurse Practitioner/ Site Physician Assistant Schedule: Monday Friday, 8:00 AM 5:00 PM Rate: $95/ Hour Hours: 40 hours per week Patient Population: Adult patients

We are seeking a Clinical Lead Nurse Practitioner to provide comprehensive primary care to adult patients while working collaboratively with primary care physicians and an interdisciplinary healthcare team.

The Clinical Lead will evaluate patients, manage acute and chronic conditions, develop individualized treatment plans, provide health education, coordinate care with specialists, and ensure accurate and timely documentation. The role also provides an opportunity to take on a leadership function by supporting care plan development and, when directed, representing the Medical Director at the assigned location.

Key Responsibilities

• Perform comprehensive patient assessments, including detailed histories and physical examinations
• Perform annual wellness assessments
• Conduct routine quarterly assessments for patients with chronic medical conditions
• Provide acute care services and support same-day access
• Assess and manage patients experiencing acute changes in condition through in-person or telemedicine visits
• Conduct post-hospitalization visits within 714 days of discharge
• Reconcile medications and develop appropriate treatment plans following hospitalization
• Coordinate care with specialists to support timely transitions of care
• Diagnose and treat common medical conditions within the scope of practice
• Manage patients with chronic illnesses and tailor treatment plans to individual needs
• Educate patients and families regarding disease prevention, health promotion, medical conditions, and chronic disease management
• Identify active diagnoses and chronic conditions to support ongoing treatment and care planning
• Develop comprehensive and accurate diagnostic lists based on clinical assessments
• Ensure documentation and appropriate diagnosis coding are completed in the EHR on the same day as the visit
• Supervise the formulation and delivery of individualized care plans across disciplines
• Monitor interventions and outcomes to ensure appropriate follow-through
• Refer patients to specialists when medically appropriate
• Maintain accurate and complete electronic health records
• Participate in quality improvement initiatives focused on patient care and outcomes
• Work collaboratively with physicians, healthcare professionals, patients, and families
• Represent or serve in the Medical Director role at the assigned location when directed or when the Medical Director is unavailable
• Practice within the scope of individual professional authority
• Complete required competencies before practicing independently
• Perform other duties as assigned

Requirements

Education

• Graduate of a Nurse Practitioner education program acceptable to NYSED, or certified as a Nurse Practitioner by a national certifying organization acceptable to NYSED

Experience

• Minimum of 2 years of clinical nursing practice in a primary care setting, home care, care management, or related field required
• Minimum of 1 year of experience working with an adult population preferred
• Candidates with less than 1 year of adult population experience may be considered if all other requirements are met and appropriate training is provided
• Strong oral and written communication skills
• Strong interpersonal skills
• High-level analytical and research skills
• Ability to manage multiple projects and meet deadlines
• Basic Microsoft Office computer skills

Licensure and Certification

• Current New York Nurse Practitioner license required
• Current New York Registered Nurse license required
• Current DEA certification required
• Current AANP or ANCC board certification required

Additional Requirements

• Must be legally authorized to practice as a Nurse Practitioner in the applicable state
• Must be medically cleared for communicable diseases
• Immunizations must be up to date before direct patient contact
• Must have a Medicaid Provider ID in good standing
• Must have a Medicare Provider ID in good standing
• Bilingual skills preferred

Physical and Functional Requirements

The position requires the ability to:

• Stand for periods of up to 6 hours per day
• Sit for extended periods, including 68 hours per day
• Lift, push, or pull up to 50 pounds of equipment, supplies, or baggage
• Safely assist with patient movement as needed
• Bend and squat when necessary for patient care and use of medical supplies
• Safely navigate stairs and walk between work areas and locations
• Travel between sites as required
• Demonstrate sufficient fine motor skills for typing and operating equipment
• Visually assess patients and accurately read orders and complete documentation
• Listen attentively and communicate clearly with patients, families, providers, and other community members
• Demonstrate sound judgment, reasoning, decision-making, and analytical skills necessary to perform the essential functions of the position
