Job Description

Experienced Nurse Practitioners are sought for per diem home health risk assessments within Polk County. This flexible, patient-focused role centers on conducting comprehensive visits for Medicare Advantage plan members living in areas such as Urbandale, Johnston, and Clive. Each visit averages about 45 minutes and may include annual health assessments or post-discharge appointments. Practitioners will work independently, providing valuable health insights without the responsibility for ongoing treatment plans or medication prescribing.

Schedule:

• Flexible shifts available between Monday and Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage plan participants in various Polk County communities
• Review and evaluate patients prior and current medical histories, conditions, and medication records
• Educate patients on medical conditions and proper medication usage during in-home visits
• Identify potential gaps in care and provide actionable recommendations for follow-up
• Focus solely on assessmentsno prescribing or treatment plan creation required

Qualifications:

• Nurse Practitioner license valid in Iowa
• MSN from an accredited school of nursing
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• At least 6 months of Nurse Practitioner experience preferred
