Take your advanced clinical expertise into the community as a Nurse Practitioner specializing in home health risk assessments. In this per diem position, youll provide essential care to Medicare Advantage members through flexible, self-directed scheduling that fits your life.

Schedule:

• Flexible shifts available throughout the week (MondaySunday)
• Set your own hours; create a personalized work schedule
• Minimum weekly or bi-weekly hour requirements

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review and evaluate prior/current medical history, health conditions, and current medications
• Deliver education to patients on their diagnoses and medication regimens
• Detect gaps in care and make recommendations to ensure optimal health outcomes
• Focus exclusively on assessment and educationno treatment planning or prescribing required

Qualifications:

• Active Nurse Practitioner license in Iowa
• MSN degree from an accredited nursing program
• Board certification through AANP or ANCC
• Current CPR certification (BLS)
• At least 6 months of experience as a Nurse Practitioner is preferred

In this role, youll travel within Polk County, which may include home visits in Urbandale, Johnston, Clive, and surrounding areas. Each evaluation generally lasts about 45 minutes, with pre-scheduled visits tailored to annual health needs or post-discharge requirements.

Compensation includes $80-$90 per evaluation, plus a mileage stipend and administrative time.
