Recovery Case Manager: Care Coordination & Discharge Prep
Key Recovery
Key Works is seeking a dedicated Case Manager to support residents in our treatment program in Seattle. You will assist patients from admission through discharge, coordinating internal and external services to promote recovery and maintain a clean and sober community. Responsibilities include arranging medical and community resources, maintaining records, and communicating effectively with clients while upholding HIPAA, 42 CFR and 45 CFR.
