POSISTION SUMMARY
Provides comprehensive medical care to a diverse patient panel, including those living with or at high risk for HIV. This role combines standard primary care management with specialized HIV prevention, diagnosis, and long-term treatment. The ideal candidate is a board-certified physician with a commitment to providing culturally competent, non-judgmental care within a medical home environment.
Practitioner is a medical practitioner who is skilled in providing high quality medical evaluation, diagnosis, and treatment to patients in a high-volume outpatient, federally qualified health center setting. This individual manages patients in accordance with standards of care and approved organizational privileges. The incumbent collaborates effectively with other health care providers, social workers, and other professionals in an integrated and evidenced based care team model approach to care. Assess and redirect emergent conditions through available emergency services consistent with organization policy. Provide afterhours consultative call on a shared, rotating basis in conjunction with afterhours triage service, as per organizational protocols.
Key Duties
- Comprehensive Primary Care: Diagnose and treat acute and chronic conditions such as hypertension, diabetes, cardiovascular disease, and obesity.
- Preventive Medicine: Perform age- and risk-appropriate screenings (e.g., STI, TB, cancer) and administer immunizations.
- HIV-Specific Care: Manage a panel of patients living with HIV, including initiating, and monitoring Antiretroviral Therapy (ART), managing virologic suppression, and treating opportunistic infections.
- HIV Prevention: Provide counseling and prescriptions for Pre-Exposure Prophylaxis (Prep) and Post-Exposure Prophylaxis (PEP).
- Patient Education: Counsel patients on treatment adherence, U=U (Undetectable = Untransmittable), harm reduction, and healthy lifestyle choices.
- Collaborative Care: Coordination with multidisciplinary teams, including case managers, mental health providers, and specialists.
- Patient Satisfaction: Collaboration with care team members is requirement of role to provide high-quality patient centered care to facilitate and maintain high patient satisfaction.
- Patient Visits : Responsible for a minimum of 1850 completed patient visits per calendar year, 154 per month, (if a full time FTE) and subject to change based on organizational needs.
- Patient Visit Notes: For optimal patient care and compliance reasons, the best practice is to close all notes within 48 hours of the visit. All notes must be completed no later than five business days of the end of calendar month when the service was provided. (Example: Notes in the electronic medical record documenting the Patient Visits in July must be entered and closed by August 5)
Care Team
- Practices at the top of practitioner's license and collaborate with care team members to provide high quality patient centered care.
- Uses data and reporting systems to regularly lead or participate in effective and routine pre-visit planning huddles with the care team.
- Communicates effectively and respectfully with all medical and operational staff concerning patient care issues and functions as a part of a Care Team, using evidence-based practices.
- Promotes teamwork, good interpersonal relations, and communication among staff at the healthcare setting.
- Actively participates in team meetings.
- Provides specialty specific consultation services upon request and within specialty scope of practice.
- Supervises clinical care, collaborates, and provides clinical consultation to PAs/APRNs, as necessary.
Qualifications
EDUCATION
- The minimum education level required for the position is a Medical Degree (MD/DO).
- Medical Degree: MD or DO from an accredited institution is required.
- MD/DO Successful completion of an ACGME or AOA-accredited residency/fellowship in internal medicine, pediatrics, family medicine, or other specialty service provided by CrescentCare under its scope of services as a FQHC and defined on the HRSA Form 5A is required.
- HIV Expertise: Minimum of 23 years of clinical experience in HIV medicine is required.
Other (Licensure, Certifications)
- Board Certification: Required in Internal Medicine or Family Medicine is required.
- Licensure: Active, unrestricted Louisiana state medical license is required.
- DEA registration in Louisiana is required.
- Current Louisiana Medical License without restriction is required.
- Board certification in the practitioner's terminal board is required.
- Maintains Basic Life Support (BLS) Certification.
Essential Physical Requirements
- Extended periods of sitting required.
- Work is primarily office based.
- Some travel may be required.
- May need to lift to twenty-five pounds in equipment and supplies.
