RN - Case Management

Day shift, 5x8 hours (07:00 AM - 03:30 PM), Monday - Friday Job Description We are seeking an experienced Registered Nurse (RN) specialized in Acute Care to join our team. This position requires a strong background in various specialties, including ICU, MS, PICU, SDU, PCU, IMC, and Obs. The ideal candidate will have previous charge experience and a commitment to quality patient care. Responsibilities Conduct thorough patient assessments and develop plan of care. Manage patient care in collaboration with healthcare teams. Perform discharge planning and needs assessments. Order durable medical equipment (DME) as needed. Adhere to regulatory requirements set by CMS. Required Experience / Certifications / Licensure Minimum 2 years of experience in an acute care setting. Active RN license required.
