LPN – LTC – Evenings

Location: Groton, NY
Position: Licensed Practical Nurse (LPN)
Setting: Long-Term Care / Nursing Home
Shift: Evenings
Shift Time: 2:00 PM – 10:00 PM
Shift Length: 8 hours
Schedule: 5 days per week, including every other weekend
Charting System: PointClickCare (PCC)

We are seeking an experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a long-term care setting. This is a full-time evening position requiring five 8-hour shifts per week, including every other weekend.

• Provide direct nursing care to residents in a long-term care environment.
• Administer medications and treatments according to physician orders and facility policies.
• Monitor residents and report changes in condition to the appropriate nursing staff or healthcare provider.
• Perform nursing assessments and accurately document resident care.
• Assist with developing and following individualized care plans.
• Collaborate with CNAs, RNs, physicians, and other members of the healthcare team.
• Maintain accurate documentation using PointClickCare (PCC).
• Follow infection-control, safety, and facility procedures.
• Provide compassionate, respectful, and resident-centered care.
• Communicate effectively with residents, families, and healthcare team members.

• Current and active LPN license required.
• Previous long-term care or nursing home experience preferred.
• Ability to work 5 days per week.
• Must be available for the 2:00 PM – 10:00 PM evening shift.
• Must be willing to work every other weekend.
• Strong clinical, communication, organizational, and teamwork skills.
• Ability to work effectively in a fast-paced long-term care environment.
