Key Responsibilities\n

• Conduct individual assessments, provide counseling, therapy, or psychotherapy based on client needs, and develop comprehensive treatment and discharge plans tailored to each patient.

• Collect and evaluate patient data through thorough interviews, detailed case histories, and observational techniques, effectively identifying root causes of their challenges to determine appropriate therapeutic approaches.

• Maintain a manageable caseload of clients requiring individual therapy sessions, while also offering collateral therapy as needed to family members or significant others, ensuring family dynamics and support systems are considered in treatment.

• Ensure meticulous documentation covering every aspect of patient care, including assessments, treatment plans, progress notes, and discharge summaries, adhering strictly to legal and compliance requirements.

• Utilize electronic health records (EHR) effectively to maintain accurate client information, ensuring congruence between physical records and digital files to meet clinical standards.

Patient Population / Client Focus\n

Your clients will primarily consist of individuals facing mental health challenges, which could include anxiety, depression, trauma, and adjustment disorders, particularly in the context of chronic illness or hospice care. Special attention will be given to understanding the unique emotional and social needs of clients in transition phases of life, particularly those at the end-of-life stage.

Qualifications\n
