Registered Nurse, Home Care

Job Summary Registered Nurse (RN) join Mass General Brigham Home Care’s Weekend Team, providing skilled 1:1 home health care for patients in the South Shore region. The role offers 20 hours per week (Saturday & Sunday) with benefit eligibility. You will lead patient care plans, deliver skilled nursing services, collaborate with interdisciplinary team members, and prevent hospital readmissions while respecting patients’ home environments. Responsibilities Lead and coordinate individualized patient care plans. Deliver 1:1 skilled nursing care in the home. Collaborate with interdisciplinary clinicians. Provide advanced clinical treatments and patient education. Help prevent hospital readmissions and improve long-term outcomes. Document care, review charts, and coordinate with LPNs, therapists, and social workers. Schedule Part time (20 hours) – Benefit eligible. Every Saturday & Sunday.

8:00 AM – 6:30 PM.

Qualifications Graduate of an approved School of Nursing. Current Massachusetts license as Registered Nurse. Minimum of one year previous nursing experience in an acute care setting (home care experience preferred). Travel within MGB Healthcare at Home geographic area required. Proficiency with computer software and hand‑held patient care devices. Strong interpersonal, customer service, and communication skills. Ability to work independently, be self‑directed, and adapt to unpredictable circumstances. Benefits & Compensation Pay range: $40.09 – $71.12 per hour. Hybrid work environment – 45 Braintree Hill, MA. Regular employee type, Day shift (United States of America).
