Role Description This is a full-time, on-site Nurse Practitioner role based in the Greater Chicago Area. The Nurse Practitioner will provide comprehensive wound care services, including assessment, diagnosis, and treatment of acute and chronic wounds in patients across various care settings. Day-to-day responsibilities include performing bedside evaluations, ordering and interpreting diagnostic tests, developing and adjusting wound care plans, prescribing medications and therapies, and documenting clinical encounters in accordance with regulatory and company standards. The role involves close collaboration with physicians, nursing staff, and facility teams, as well as educating patients and caregivers on wound prevention, dressing changes, and overall skin health. The Nurse Practitioner will also participate in quality improvement initiatives, follow infection control protocols, and support care coordination and transitions to reduce complications and rehospitalizations. Qualifications Active Nurse Practitioner license in the state of Illinois and national board certification (AGNP, FNP, or equivalent). Demonstrated clinical skills in wound assessment, debridement, dressing selection, infection management, and overall wound care best practices. Experience working in long‑term care, home health, skilled nursing facilities, or similar patient care environments is highly beneficial. Strong skills in clinical documentation, use of electronic health records, and adherence to regulatory and compliance requirements. Excellent interpersonal and communication abilities to collaborate with interdisciplinary teams and to educate patients and caregivers. Ability to travel between facilities in the Greater Chicago Area, manage a mobile schedule, and work independently in on‑site settings. Commitment to patient‑centered care, cultural sensitivity, and maintaining high standards of professionalism and ethics.
