About the Position

Leadership experience preferred. License: RN Licensed as a Registered Nurse in the State of New York required. Certifications: Baccalaureate degree required. PRI and Screen certification within three months of employment is required. Master’s Degree and Bilingual Spanish Certification in Case Management preferred. Must-Have: - Hospital experience - Bilingual (Spanish speaking) - Care coordination experience - Review assessments Description: The Care Manager develops, facilitates, monitors, and communicates a care plan in partnership with the member, their family, or significant other, primary caregiver, and various providers. Using assessments and interviews conducted by the Assessment Nurse, the Care Manager identifies the risk factors, strengths, challenges, and service needs of the member while striving to keep them safe and well in their community setting. The role involves reviewing and evaluating assessments and information for members in the managed long-term care and other lines of business. The Care Manager will develop a working relationship with the primary care provider to discuss the care of the member. Assessment findings will be reviewed with the primary care provider to identify any concerns that have not been identified by the clinical team. Risk factors will be identified, and a risk category will be assigned to the member. As part of the Care Management team, a formal care plan will be developed for all services needed for the member, including the member's disaster plan. The condition of all members will be monitored at least monthly, typically by telephone but face-to-face when necessary. Clinical issues requiring immediate assessment and/or treatment will be identified to reduce the risk of unnecessary hospitalizations or nursing home admissions. Opportunities to improve the quality of care will be identified by ensuring members receive needed preventive and chronic disease care. Requests for additional services will be approved based on assessments and using evidence-based standards, with denials, reductions, or limitations referred to the Medical Director. Assistance will be provided to members with the coordination of services both within and outside networks as appropriate, including facilitation of discharge from acute settings. The Care Manager will provide care coordination through the continuum of care, optimizing both the quality of care and the quality of life for the members. Coordination with the Utilization Management department will occur on concurrent and retrospective reviews. Follow-ups will be conducted with assigned nurses for clinical updates to care plans. Documentation will be completed within two business days of coordination notes and routine contacts with members based on their risk level. Participation in team care planning meetings is expected. Complaints that can be resolved in one day will be handled efficiently. Cooperation with all departments is essential. Members appropriate for specialty programs will be identified. All management activities will be performed in compliance with regulatory agency requirements. Information will be provided in response to requests from the Quality Management Department to be reviewed by various Quality committees. Completion of all other tasks assigned by department leadership is expected. Participation in the department on-call schedule will be necessary, with rotation amongst team members.
