About the Position

Job Title: Care Manager Profession: Care Manager Specialty: Managed Long Term Care Duration: Full-time Shift: Monday to Friday Hours per Shift: 9 AM to 5 PM Experience: Five years minimum of clinical experience as a Registered Nurse; Leadership experience preferred; Two to three years clinical experience in home health agency or managed care program. License: RN Licensed as a Registered Nurse in the State of New York required. Certifications: Baccalaureate degree required; PRI and Screen certification within three months of employment is required; Master's Degree and bilingual certification in Case Management preferred. Must-Have: - Experience with Care Coordination, Utilization Review, and discharge planning. - Computer literacy required. Description: The Care Manager develops, facilitates, monitors, and communicates a care plan in partnership with the member, their family or significant other, primary caregiver, and various providers. The Care Manager uses assessments and interviews to identify the risk factors, strengths, challenges, and service needs of the member. The goal is to keep members safe and well in their community setting. The Care Manager reviews and evaluates assessments for members and develops a working relationship with primary care providers. They monitor the condition of all members at least monthly, typically by telephone and face-to-face when necessary. The Care Manager identifies clinical issues that require immediate assessment or treatment. They help improve the quality of care and ensure members receive needed preventative and chronic disease care. The Care Manager assists members with the coordination of services within and outside of networks. They provide care coordination through the continuum of care and optimize both the quality of care and quality of life for members. The Care Manager documents coordination notes and routine contacts within two business days. They participate in team care planning meetings and handle complaints that can be resolved quickly. The Care Manager cooperates with all departments and identifies members appropriate for specialty programs. They perform all management activities in compliance with regulatory agency requirements and provide information for review by quality committees. Other tasks may be assigned as needed. The Care Manager participates in the department on-call schedule, which is rotated among the care team.
