Nurse Case Manager: Catastrophic Care & Care Coordination

TRIUNE Health Group is seeking a Nurse Case Manager to coordinate resources and create cost-efficient options for catastrophically or chronically ill individuals, facilitating individualized treatment goals and timely return to work when appropriate. The role involves collaboration with patients, physicians, employers, and providers to assess needs, ensure medical necessity, and manage care within HIPAA guidelines.
