Are you seeking a full time opportunity with flexible hours?We are currently seeking a Nurse Case Manager for our Manhattan Residences.Responsibilities include:Coordinate the delivery of health care services for adults with disabilities residing in residential programs.Be responsible for performing nursing assessments and diagnosesWork in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.Provides guidance and training to direct care staff to ensure proper delivery of health care services.Requirements include:NYS RN LicensePrior nursing experience and/or experience working with persons with developmental disabilities preferred.Excellent oral and written communication skills.Ability to travel to and access multiple locations throughout the metropolitan area.Able to work flexible hours and/or days.We offer a competitive salary and an enviable list of comprehensive benefits which includes four weeks' vacation, personal and holiday days, tuition reimbursement, license renewal and/ conference/workshop reimbursement, medical, dental, life insurance, and 403(b).
