About the Position

License: Valid state license must be in hand at submission. Certifications: Must have acute care experience. Must-Have: Experience in case management within the last year. Experience with HMOs, IPAs, advance directive, POLST, and protective services process is required. Description: The role requires a registered nurse specializing in case management. The individual will be responsible for care coordination and discharge planning. Candidates must be able to float to meet departmental needs based on patient care requirements. The case manager will manage a caseload of up to 25 patients. Experience with admissions criteria, appeals and denials, benefits eligibility, and prior authorizations is necessary. Knowledge of CMS guidelines and HIPAA regulations is essential. Candidates must commit to working every other weekend. Travelers must provide references from previous employment covering one year of experience within the last three years. All requests for time off must be submitted upfront during the application process. Flexible scheduling and the ability to adapt to changes in the work environment are crucial. Candidates will be expected to work closely with interdisciplinary teams to ensure quality patient care. A strong understanding of DME needs assessment and worker's compensation case management is beneficial.
