This is an exceptional opportunity to practice at the top of your license in a specialized outpatient setting, caring for patients with advanced heart failure while collaborating with heart failure physicians and a multidisciplinary team dedicated to delivering exceptional patient care. As our Heart Failure program continues to expand, this position offers the opportunity to contribute to program development and quality initiatives, with the potential for future advancement into an APP lead role supporting multiple ambulatory cardiology clinics. Why Join our Team Four 10-hour weekday schedule No weekends, holidays, or call responsibilities Specialized outpatient practice focused on advanced heart failure management High level of APP autonomy within a collaborative academic environment Work alongside Advanced Heart Failure cardiologists Participate in quality improvement and clinical research initiatives Build lasting relationships with patients through continuity of care Opportunity for professional growth and future APP leadership What You'll Do As a Physician Assistant II, you will play a vital role in the ongoing management of patients with advanced heart failure by: Independently evaluating, diagnosing, and managing patients in the Heart Failure Bridge Clinic. Developing individualized treatment plans while optimizing guideline-directed medical therapy. Assessing volume status and initiating appropriate interventions, including IV diuretic therapy. Managing patients with complex cardiovascular conditions in collaboration with Advanced Heart Failure physicians. Coordinating transitions between inpatient and outpatient care to improve outcomes and reduce hospital readmissions. Educating patients and families on heart failure management, medications, lifestyle modifications, and symptom monitoring. Collaborating with physicians, nurses, pharmacists, case managers, and other multidisciplinary team members to deliver comprehensive, patient-centered care. Supporting evidence-based practice, quality improvement initiatives, and established clinical protocols. Documenting and billing for professional services. Qualifications Required Graduate of an accredited Physician Assistant program. NCCPA certification. Current Maryland Physician Assistant license (or eligibility). Minimum of two (2) years of recent cardiology experience as a practicing Physician Assistant. Experience managing patients with cardiovascular disease in an in-patient and/or outpatient setting. Strong clinical judgment, critical thinking, and decision-making skills. Excellent communication and interpersonal skills with a commitment to collaborative, patient-centered care. What do we offer? Salary Range: Minimum 60.93/hour - Maximum 94.45/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
