Active Illinois COTA license or ability to obtain state licensure prior to start of assignment. BLS certification from AHA or ARC may be required prior to start. Experience working in skilled nursing facilities preferred. Experience providing restorative and daily living skills training for geriatric or post-acute populations preferred. Ability to follow treatment plans under the supervision of a licensed OT and document in electronic medical records. Assignment Details Facility Type: Skilled Nursing Facility. Shift Name: Days. Shift Details: Days 08:30–16:30. Work Pattern: Five 8-hour shifts per week. Start: ASAP. Contract Length: 13 weeks.
