\nFairview is seeking a Physician Assistant to join our primary care practice at M Health Fairview Clinic River Falls . Join a collaborative and growing practice with a supportive, experienced care team dedicated to providing high-quality, patient-centered care.
\n \nPractice Details:
\n- \n
- Established primary care practice with a collaborative team-based environment \n
- Patient encounters average 18 patients per day \n
- 1.0 FTE = 34 patient contact hours per week \n
- Openings: 0.8 to 1.0 FTE (Full Time) flexible scheduling options \n
- Fully integrated Electronic Medical Record (EPIC) \n
Responsibilities
- \n
- Systematically assesses patients health status, arrives at appropriate diagnosis(es), and implements care plan for effective clinical outcomes. \n
- Performs appropriate medical care within scope of practice as defined by practice agreement with the supervising physician. \n
- Considers thorough range of treatment/preventive options and arrives at a recommended course of action that are age appropriate, culturally appropriate, realistic and attainable. \n
- Appropriately manages patient care and health status following initiation of treatment/preventive plan, and monitors and adjusts as warranted. \n
- Communicates effectively with patients and family so that they understand their diagnosis, treatment plans and need for follow-up care. \n
- Educates and encourages patients to become an active participant in their own health care related behavior and provides patient education materials for use in encouraging wellness. \n
- Seeks consultation and advise by providing adequate information to supervising physicians and communicates requests effectively, or responds to such requests from the referral source in a courteous, professional, and timely manner. \n
- Documents according to established guidelines to facilitate ongoing quality of care in a timely manner. \n
- Delegates direct and indirect patient care activities to appropriate personnel. \n
- Responds appropriately to emergent situations. \n
- Maintains and improves skill level through participation in continuing medical education. \n
- Actively participates in clinical quality improvement initiatives. \n
- Reviews and follows-up on complaints and concerns about medical care. \n
- Takes actions to assure continuity of care for patients. \n
- Contributes to effective and efficient practice management policies and practices that support patient access to medical care. \n
- Participates in access planning for the clinic site, ensuring appropriate amount of same day patient access. \n
- Meets number of clinic patient contact hours as outlined in clinic policies. \n
- Collaborates in establishing personal numbers of relative value units to be budgeted and achieves targeted goal. \n
- Fulfills commitments for call assignments with clear communication and follows through on work initiated. \n
- Supports appropriate billing for professional service through timely documentation and maintaining adequate knowledge of FVC billing policies and procedures. \n
- Adheres to all relevant policies, procedures and practice guidelines. \n
- Assists with the preparation for JCAHO and other regulatory surveys. \n
- Responds to telephone calls in a timely fashion. . \n
- Manages patient care through appropriate use of available resources. \n
- Appropriately prescribes and orders ancillary services. \n
- Utilizes Fairview referral network appropriately. \n
- Promotes the appropriate length of stay for patients. \n
- Manages chronic health/disease conditions. \n
- Provides leadership for clinic and community relations. \n
- Serves as a resource to colleagues and support staff, enhancing quality of care. \n
- Participates in the training, orientation, and evaluation of clinic support staff. \n
- Regularly attends and actively participates in clinic staff meetings, MPOC or Physician/AP staff meetings. \n
- Participates in appropriate committee structure. \n
- Participates in community education and other professional activities to promote Fairview Health Services and the site. \n
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served. \n
Required Qualifications
- \n
- Graduate of an accredited Physician Assistant training program. \n
- BC/BE through NCCPA. \n
- Licensed or ability to obtain a PA license with prescriptive authority through the Minnesota Board of Medical Practice. \n
- Ability to obtain and /or maintain DEA certification for the State of Minnesota. \n
- \n
Licensed or ability to obtain licensure as a Physician Assistant through the Wisconsin Physician Assistant Affiliated Credentialing Board.

Ability to obtain and/or maintain DEA certification for the State of Wisconsin.
\n - Meet threshold criteria/qualifications for Credentialing and Privileges (if required). \n
