Wellbeing for Life Practitioner

Overview The Wellbeing for Life (WBFL) service is a commissioned service in County Durham delivered by a partnership between County Durham and Darlington NHS Trust (CDDFT) and the Pioneering Care Partnership (PCP). It is based on the principle of Your Wellbeing, your way, using behaviour-change approaches to help people living and working across County Durham take small steps to better health in a way that suits them. Responsibilities Engage with a wide range of partners and local voluntary and community groups to develop joint health improvement activities, ensuring specific groups are reached by the service. Carry a caseload of clients aged 16 years and over to facilitate lifestyle and behaviour change in areas such as weight management, smoking, mental wellbeing and reducing alcohol intake. Promote engagement with the service and provide one-to-one and group wellbeing advice and support to clients, including signposting to relevant NHS and non-NHS services. Work as an integrated member of a team with a flexible approach, across the geographical location/patch to meet the needs of the local community.
