Description

• Assess and evaluate patients in either inpatient or outpatient contexts, crafting personalized treatment plans
• Deliver physical therapy interventions centered on mobility, strength, balance, and functionality
• Engage in teamwork with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Educate patients and families on therapeutic objectives, discharge planning, and home exercise routines
• Maintain documentation of evaluations, treatments, and progress according to facility policies and regulations
• Participate in interdisciplinary team discussions and care rounds to guarantee cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Prior experience is strongly preferred
• Capacity to treat patients with multifaceted medical and mobility challenges
• Excellent communication and teamwork capabilities
