Duties

Manage a caseload of patients with chronic conditions by assessing needs and creating individualized care plans.

Coordinate care across providers, clinics and community programs to ensure patients receive timely services and care.

Provide education and coaching to patients and families to promote understanding, self-management, and treatment adherence.

Collects and maintains case management data, including service utilization, identified barriers, interventions, and patient outcomes.

Use patient data to identify care gaps, improve coordination processes, and support quality improvement efforts.
