Care Navigator & Outreach Specialist (Hybrid - RVA)

Waymark seeks an Outreach Specialist / Care Coordinator for the Greater Richmond area. You will be the frontline liaison with healthcare facilities and patients, building relationships with primary care physician partners while improving access to resources. You will collaborate with pharmacists, social workers, and community health workers to coordinate care and document encounters. You will work in a hybrid role, traveling within the region, attending a short paid onboarding training, and #J-18808-Ljbffr Waymark
