Field Care Manager Nurse

Become a part of our caring community Humana Gold Plus Integrated is seeking a Field Care Manager Registered Nurse (RN) to assess and evaluate member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members and families toward resources appropriate for the care and wellbeing of members. The Field Care Manager Nurse employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues. The Field Care Manager Registered Nurse key roles and responsibilities may include the following: Identifiesand resolves barriers that hinder effective care. Ensurespatient is progressingtowards desired outcomes by continuouslymonitoringpatient care through assessments and/or evaluations. Createsplansofcare. Communicates with internal and external stakeholders. Understands department, segment, and organizational strategy and operatingobjectives, including their linkages to related areas. Makes decisionsregardingown work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows establishedguidelines/procedures. VisitsMedicaid members in their homes,SupportiveLiving Facilities, and/or Long-Term Care Facilities and other care settings – 75-90% local travel (see Additional Information section). Upon hire, this role will be assigned to support either MLTSS or LTSS based on business needs. Use your skills to make an impact Required Qualifications Must reside in the state of Illinois. Registered Nurse in the state of Illinois without disciplinary action. 2+ years ofexperience ofin-home case/care management. Experience working with the adult population. Knowledge of community health and social service agencies andadditionalcommunity resources. Exceptional communication and interpersonal skills with the ability to quickly build rapport. Ability to work with minimal supervision within the role and scope. Ability to use a variety of electronic informationapplications/softwareprograms including electronic medical records. Intermediate toAdvanced computer skills and experience with Microsoft Word, Outlook, and Excel. Excellent keyboard and web navigation skills. Preferred Qualifications Bachelor of Science in Nursing (BSN). 3+ years of in-home assessment and care coordination experience. Experience with Medicare/Medicaid members. Experience with health promotion, coaching, and wellness. Previousmanaged care experience. Bilingual — English, Spanish. Certification in Case Management. Motivational Interviewing Certification and/or knowledge in area. Additional Information Workstyle/Travel: This is a Hybrid–Home position that requires occasional onsite work atthemarket office in Schaumburg,Illinois, aswell75-90% travel in the field to visit membersthroughout Illinois. This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information. Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $71,100 - $97,800 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
