Minimum Requirements

Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
Active Physical Therapist license in Indiana required prior to the start of assignment.
BLS certification from AHA or ARC may be required.
Home health experience preferred. (Preferred)
Experience conducting SOC (start of care), evaluations, and treatment of physical therapy eligible patients in a home health environment.
Familiarity with home health documentation systems such as HCHB or PointCare is preferred. (Preferred)
Ability to travel within and provide visits across service areas including Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells and Whitley counties.
Minimum education: Bachelor's Degree.

Assignment Details

Facility Type: Home Health.
Shift Name: Days.
Shift Hours: 08:00–16:00 (Day shift).
Weekly Hours Guarantee: 36 hours per week.
Coverage Areas: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells and Whitley Counties.
Mileage reimbursement provided between visits, from first patient to last patient.
On-call/weekend requirement: None.
Primary responsibilities: SOC, evaluation, and treatment of PT-eligible patients in the home health setting.
Required license: Licensed Physical Therapist in Indiana.
