Work Environment\n

This role is predominantly remote, allowing for flexibility while still ensuring that our clients receive the best support possible. You will collaborate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to deliver comprehensive care. Your schedule will predominantly focus on weekdays, with operational hours from Monday to Friday, 8:00 AM to 4:30 PM. However, occasional evenings or weekends may be necessary to meet client needs.

Key Responsibilities\n

• Perform thorough assessments to identify seniors' psychosocial needs, taking into account emotional, mental, and social factors.

• Develop and implement individualized care plans in collaboration with clients and their family members, ensuring their specific goals and preferences are addressed.

• Provide counseling and emotional support to seniors, guiding them and their families through significant life transitions and challenges.
\n
• Advocate for clients’ access to community resources and rights, ensuring they receive appropriate social services.
\n
• Engage in regular visits to monitor clients' overall well-being, evaluate their living situations, and address any safety concerns.
\n
• Facilitate open communication among family members, helping to resolve conflicts related to caregiving responsibilities.
\n
• Educate clients and family members on available resources, support networks, and activities designed to enhance social engagement.
\n
• Maintain precise documentation of client interactions, progress notes, and care plans using electronic health record (EHR) systems.

• Stay current with developments in social work practice, policies, and ethical standards, ensuring compliance across all client interactions.

Patient Population / Client Focus\n

Your primary focus will be working with seniors and their families, assisting them in navigating the various challenges associated with aging. You will specifically address issues such as mental health, access to services, and the social determinants of health that impact their quality of life. By recognizing and advocating for the needs of vulnerable populations, you will ensure they receive the holistic support necessary for their well-being.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in social work, with a preference for experience in geriatric or community health settings.

Compensation and Benefits\n
