We are hiring an Admission Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the South Park , PA area. This position is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid within the first 18 months of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!

What You’ll Do:

\n

\n
• \n

  Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

\n

\n

\n
• \n

  Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders

\n
• \n

  Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one

\n
• \n

  Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation

  Initiate evaluations to other disciplines when indicated

• \n

  Work independently, but never alone

  Travel to patient homes within assigned territory

Admission RN Qualifications

\n

\n
• \n

  Active Registered Nurse, RN license in the state you work

\n
• \n

  One year of RN experience and the clinical expertise to deliver quality patient care

\n
• \n

  Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
