Field Health Care Case Manager - Miami, FL (170388) Pay: $34.13 per hour Schedule: Monday–Friday, 8:00 AM–5:00 PM EST Location: Work from home + 50–75% travel for face-to-face member visits in Miami Candidate must reside in Miami Dade County, FL (MUST BE IN ONE OF THESE ZIP CODES. 33192, 33182, 33172, 33193) Summary Field Health Care Case Manager role supporting Medicaid Long Term Care/Comprehensive Program members. Responsible for telephonic and face-to-face assessment, care planning, care coordination, and documentation, collaborating with providers and using care management tools to support medically complex populations. Requirements Fluent bilingual Spanish/English (read, speak, write) — required 1+ year case management experience — required Bachelor’s degree required (social work or related field preferred); no nurses Comfortable with 50–75% travel for member visits (home/assisted living/nursing homes) Proficient with Microsoft Word, Excel, Outlook Strong communication skills; able to multitask and work in a fast-paced environment Long-term care experience (preferred) Case management certificate (preferred) #J-18808-Ljbffr A-Line Staffing Solutions
