Post Acute Care Physician / Primary Care Physician - Providence, RI

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long‑term care, and assisted living facilities. Deliver face-to-face primary care services and participate in educational sessions and clinical reviews. Collaborate with nurse practitioners and physician assistants to enhance patient care. Serve a diverse patient population, from short‑term rehab to complex geriatric cases, aiming to improve access and clinical outcomes.
