A respected Federally Qualified Health Center is seeking a Primary Care Provider – Adult Medicine to join its outpatient team serving the adult community in Providence. Position Overview

The Primary Care Provider will provide comprehensive outpatient care to adult patients in a collaborative, mission-driven setting. This role focuses on preventive care, chronic disease management, wellness services, and improving overall health outcomes for a diverse patient population.

Responsibilities

• Provide comprehensive outpatient primary care services to adult patients
• Diagnose and treat acute and chronic medical conditions
• Conduct annual wellness exams and preventive screenings
• Manage chronic diseases such as diabetes, hypertension, and cardiovascular conditions
• Coordinate referrals to specialty care when appropriate
• Maintain timely and accurate EMR documentation
• Collaborate with interdisciplinary care teams to support whole-person care

Qualifications

• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Active or eligible Rhode Island medical license
• DEA certification or ability to obtain
• Experience in outpatient primary care, community health, or FQHC preferred
• Commitment to serving diverse and underserved adult populations
