ACT/FACT Co-Occurring Specialist - Pottstown

Hours: Full-Time, 40 hours per week Responsibilities Meet with ACT participants in their homes and other community venues to deliver in vivo services that improve quality of life and maintain community tenure. Assist with activities of daily living, budget planning and money management, housing maintenance, accessing benefits and entitlements, meal planning and grocery shopping, reliable transportation, side‑by‑side coaching and training, and transport to community activities or appointments. Provide medication services, including delivery and observation of ingestion. Coordinate services with ACT staff and develop, implement, evaluate, and revise treatment goals. Provide individual and supportive counseling for symptom management. Provide on‑call crisis services on a rotating schedule and cover program needs. Complete documentation of all services in the electronic health record on time. Work weekend, evening, and holidays on a rotating schedule to cover program needs. Cross‑train with other ACT specialties to provide additional services such as housing or vocational support. Assess each participant’s history of substance use, develop a treatment plan, facilitate admissions to higher levels of care, and communicate with treatment staff in those facilities. Provide one‑on‑one psychotherapy in the field using evidence‑based strategies such as Motivational Interviewing, Dialectical Behavior Therapy, and Integrated Dual Disorders Treatment. Use harm‑reduction techniques and other strategies that align with participants’ current stage of change. Provide group psychotherapy using the Integrated Dual Disorders Treatment model to address symptoms and build skills for symptom management. Provide primary support to ACT participants and cross‑train team members to promote stability and increase community tenure. Perform any other duties assigned. Qualifications and Experience Master’s degree in social work, psychology, counseling, or a related, license‑eligible field. Minimum two years of direct‑service experience with individuals with substance use disorders. Experience working with individuals with forensic involvement and/or severe and persistent mental illness is preferred. Current CAADC certification from the Pennsylvania Certification Board. ASAM certification preferred; must obtain within one year of hire. Valid driver’s license and compliant driving record in accordance with St. Luke’s University Health Network policy. Ability to perform heavy work, lift and carry up to 50 lbs, stand, walk, and sit for at least 6 hours in an 8‑hour day. Good physical and mental health, with required visual and auditory acuity. Physical & Sensory Requirements Heavy work exertional activity, lifting up to 50 lbs, and sustained sitting, standing, or walking for at least 6 hours in an 8‑hour day. Stooping, crouching, grasping, and fine manipulation as required. Functional capacity to perform medium, light, and sedentary work.
