Family Practice/Primary Care Physician Assistant

Conviva Senior Primary Care is seeking an experience clinician for our Float Nurse Practitioner/Physician Assistant position in Pinellas County, Florida for mainly two locations.

As a clinician with Conviva, you ll have the opportunity take a hands-on approach to addressing patient needs inside the exam room, as well as looking beyond to understand how social determinates of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a multidisciplinary approach.

A day in the life of a Conviva provider:

• See and average of 15 patients per day. Opportunity to spend more time with patients
• Thrive in a value-base care model and improve patient s quality of life
• See a variety in your caseload, from preventative care to chronic conditions
• Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care

Required Qualifications:

• Two years' experience as a nurse practitioner/physician assistant seeing patients 65+ and or managing chronic conditions
• Active, unrestricted NP/PA license in Florida or to obtain one
• Current certification as an NP or PA
• Current DEA license or ability to obtain a DEA license
• If selected for this role, you will be required to be screened for Tuberculosis.

• Familiar with HEDIS measures
