Home Health Nurse Practitioner (NP) - Eddyville, Iowa
Step into a rewarding opportunity as a Nurse Practitioner providing home health risk assessments within Wapello County. This flexible, part-time position centers on conducting comprehensive evaluations for Medicare Advantage members, focusing on annual assessments and post-discharge visits. Each assessment typically requires about 45 minutes, and visit locations may vary across communities, including Agency, Blakesburg, Chillicothe, Eldon, and Kirkville.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to fit your lifestyle
- Minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete thorough home health risk assessments for Medicare Advantage plan members
- Review and evaluate past and current medical and medication histories
- Offer education related to chronic health conditions and medications
- Identify and report care gaps, recommending steps to improve patient outcomes
- Please note: No treatment planning or medication prescribing is involved
Qualifications & Experience:
- Valid Nurse Practitioner license in Iowa
- MSN degree from an accredited nursing program
- Board Certification (AANP or ANCC)
- Current CPR certification (BLS)
- At least one year of Nurse Practitioner experience preferred, though new practitioners are welcome to apply
